You must fill in this attached statement at the start of the course. If you can answer NO to all the points on page 1 then you do not need a medical certificate from a doctor. If you can answer a YES to questions 3, 5 or 10 OR to any of the questions on page 2 in the corresponding section to the YES on page 1 then you will need to get page 3 filled in by your doctor before starting the course.

The course price includes: SSI Digital Student Materials, 4 speciality courses (includes 8 dives - 5 of these dives are from our boat in Puerto Del Carmen), Photos of you underwater, Manta Diving T-shirt, Equipment hire, Insurance for duration of the courses, SSI Advanced Open Water printed card.
