Insurance:
In Spain it is required by law to have insurance that covers you for scuba diving for personal accident and search and recovery. If you have your own insurance you need to bring with you the policy certificate and wording.
Medical:
You must fill in this attached statement at the start of the course. If you can answer NO to all the points on page 1 then you do not need a medical certificate from a doctor. If you can answer a YES to questions 3, 5 or 10 OR to any of the questions on page 2 in the corresponding section to the YES on page 1 then you will need to get page 3 filled in by your doctor before starting the course.
